At Primary Care Associates of Appleton, we see patients by appointment only. Same day appointments are available for urgent or sudden illness or injury. Please call our Patient Services department at (920) 996-1060 to schedule an appointment anytime during our regular office hours.

If you are unable to keep an appointment, please contact our office as soon as possible so the time can be made available to other patients; one of our Patient Services Representatives will be happy to assist in rescheduling your appointment to a more convenient time. Please know our policy requires 24 hours notice for cancellations, and a fee may be assessed for a missed appointment.
