MyChart ensures confidentiality and security by only allowing access via a password protected account, of which only you know the login information. Also, communication exchanged through MyChart is encrypted, offering confidence that your personal medical information is not being intercepted and read by others.

New Users
For access to a family member’s health record (e.g. aging parent or child), please click here to access and print the appropriate “proxy access” form. The proxy access form will need to be returned to our office prior to receiving access. Please know that once a child turns 12 years of age, a parent/guardian will have limited access to the child’s health record due to state laws protecting the child.
If you would like access to MyChart, please contact our office at 920-996-1000 to receive an activation code. Upon receiving your activation code, you will have 60 days to visit MyChart to activate your account; the code will expire after 60 days and a new one will need to be requested. (If you are here for a visit, an activation code will also be provided on the “After Visit Summary” provided to you after your appointment by your healthcare provider team.)
Please note: Primary Care Associates of Appleton uses ThedaCare’s Epic electronic health record system. As a result, you may see the names “MyChart” and “MyThedaCare” used interchangeably. They are the same patient portal, and you do not need to create separate accounts. When prompted to select your healthcare organization or MyChart provider, select ThedaCare.
Even without an Activation Code, you can still sign up for MyChart! Visit the link https://my.thedacare.org/MyChart/Signup, enter your demographics and your identity will be verified using questions from a third-party verification system. Once verified, you will be able to create your MyChart/MyThedaCare username and password. If you have any questions, please contact us.
