What started as a vision to provide continuous, comprehensive, family-centered healthcare came together when a group of eight physicians opened the doors of Primary Care Associates of Appleton on September 5, 2000 at 620 E Longview Dr. in Appleton (formerly known as the old “Doctors Park”). Those doctors, pictured above include: (back row, left to right) Robert M. Makeever, Robert S. Fox, David M. Ebben, D. Jon Derksen, Dan L. Heyerdahl, and (front row, left to right) Jonathan E. Hagen, Amy H. Servais, and Charles A. McKee.  

Primary Care Associates of Appleton was established in response to patient and community needs. Being employed physicians for over five years, the group wanted to return to independent practice to maintain focus on their patients and the unique qualities and individuality of each patient. The group also wanted to foster and build on the relationships that should be established with patients, getting back to the primary role of being a patient advocate in all things.

From that point on, Primary Care Associates of Appleton has continued to grow, welcoming new providers and services as they best fit within our "family" culture.  We are here to help the patient, their family and the community, and are committed to provide the services necessary to improve and maintain their quality of life.
