Appointments are required. Call (920) 996-1060 to schedule. Patients should enter our parking lot from Intertech Court to collect check-in paperwork, and then proceed to drive under our facility awning to receive your vaccine and proceed on your way. We encourage patients to wear a short-sleeve shirt to facilitate progression through the drive-thru line.
